What is a CDL Medical Certificate

What is a Commercial Driver License (CDL) Medical Examiner's Certificate

Commercial drivers, firefighters, and drivers with Class A or B licenses must meet medical standards in order to drive commercial motor vehicles. 

Medical Examiner's Certificate (MEC) Form MCSA-5876

Once the Medical Examiner determines that the driver is physically qualified to drive a commercial motor vehicle in accordance with the physical qualification standards, the Medical Examiner completes and provides the driver with a Medical Examiner's Certificate (MEC), Form MCSA-5876. This is what most of us call the 'DOT Medical Card'. The Medical Examiner will keep a copy the Medical Examiner's Certificate on file for at least 3 years.

Medical Certificate Renewals

 If your medical examiner's certificate is about to expire, you can send your updated certificate to the Texas Department of Public Safety via email, fax, or mail.  You may send in your medical examiners certificate for processing only if your Commercial Driver License or Commercial Learner Permit has not been downgraded.

The Medical Examiner's Certificate must be sent as an attachment, it cannot be sent as a link.

If you submit by fax, please fax your MEC to 512-424-2002.

If you would like a confirmation receipt for your records, be sure to set your fax to accept receipts before sending over fax.

Checking Status:

You may check to ensure your Medical Certificate has been processed by accessing the License Eligibility Application at https://txapps.texas.gov/txapp/txdps/dleligibility/login.do Records may take up to 10 days to be updated.

Commercial Driver License Downgraded for Medical Certification

If your CDL or CLP has been downgraded due to an expired Medical Examiner's Certificate, you must visit your local driver license office to be upgraded. A CDL that was downgraded to a driver license (DL) solely for failure to provide a valid medical variance or medical examiners certificate, may be upgraded back to your previous CDL status (including restrictions and endorsements) without taking CDL knowledge and skills exams provided the following conditions are met:

  • The record status must be Eligible (NOT suspended, revoked, disqualified, canceled)
  • The DL has not been expired for more than two years
  • The CDL downgrade occurred within the last 12 months
  • You must meet all CDL requirements and provide valid medical variance or medical examiners certificate (if applicable) or certify to an excepted status
  • Applicable transaction fees will apply
